There are many things about "Sweetheart" that just don't work. First, the book hardly stands on it's own. If you haven't read "Heartsick" then much of the characters, plot, and motivations of this story will make no sense. Sweetheart is less gruesome than Heartsick, but it is more psychologically bizarre and unbelievable. Instead of graphic torture, there is graphic sex. The pace of action in Sweetheart is less intense, and there are even more improbable coincidences than in Heartsick. Perhaps borrow this one from the library if you really want to read the followup to Heartsick, but otherwise this one probably isn't worth your time.
